How to Apply

• Fill out the ONLINE FORM or download and complete the Member Request for Service Form
• Attach receipts – receipts will be returned to you upon request
• You can email your form and attach receipts and send to info@hemophiliamb.ca or mail the form and receipts to the Chapter office.
• NOTE: All Client Service claims must be made within six months of receiving the service.
